This Merge Label Template will adhere to the Avery 5164 Label format and is used to produce merge envelopes based on a data document:

• Data source Information
  Choose to create a data document or retrieve an existing data document
  Choose to include/exclude the firm's logo
  Choose to include/exclude the bar code

When choosing to create a new data document, the wizard will query the DB for reports based on your selection.
