• Provide the name of the client in the Client Name text box.

• Provide the matter number in the Matter No. text box.

• Provide the name of the case in the Case Name text box.

• Provide the case number in the Case No. text box.

• Provide the number of the file in the File text box

• Provide the total number of files in the of text box.
